Esomeprazole
Indication
Gastric antisecretory treatment when oral route is not possible eg, GERD in patients w/ oesophagitis &/or severe symptoms of reflux. Healing of gastric ulcers associated w/ NSAID therapy. Prevention of gastric & duodenal ulcer associated w/ NSAID therapy in patients at risk. Prevention of rebleeding following therapeutic endoscopy for acute bleeding of gastric or duodenal ulcers.
Instruction
Nexium inj Reflux oesophagitis 40 mg once daily. Symptomatic treatment of reflux disease 20 mg once daily. Healing of gastric ulcers w/ NSAID therapy 20 mg once daily. Prevention of gastric & duodenal ulcers associated w/ NSAID therapy 20 mg once daily. Inj: 20 or 40 mg IV over 3 min. Infusion: 20 or 40 mg IV infusion over 10-30 min. Prevention of rebleeding of gastric & duodenal ulcers 80 mg as a bolus infusion over 30 min, followed by 8 mg/hr continuous IV infusion over 3 days. Hepatic impairment Max dose: 20 mg daily.
Drug interaction
May increase serum levels of tracrolimus, methotrexate & change absorption of protease inhibitors eg, atazanavir, nelfinavir & saquinavir. May prolong elimination of drug metabolized by CYP2C19 eg, citalopram, imipramine, clomipramine, phenytoin, diazepam. May decrease absorption of ketoconazole & itraconazole. Warfarin, voriconazole, cisapride, clarithromycin.
