Diazepam
Indication
Motor excitation of diverse etiology & paranoid-hallucinatory states. Status epilepticus; tetanus. Relief of severe spasm of central, peripheral, rheumatic or tetanic origin.
Instruction
Adult States of excitation 10-20 mg IM/IV tds. Status epilepticus Initially 10-20 mg IV, followed by 20 mg IM or IV drip infusion. Muscular spasm 10 mg IV once or bd. Tetanus 10 mg IV for 8 hr. Elderly Antianxiety, sedative-hypnotic, amnestic, anticonvulsant or adjunct skeletal muscle relaxant Initially 2-5 mg IM/IV per dose, may be increased gradually as needed. Childn 2-5 yr Status epilepticus & severe recurrent convulsive seizures 200-500 mcg slow IV every 2-5 min, may be repeated in 2-4 hr as needed. Max: 5 mg. Tetanus 1-2 mg IM/IV every 3 or 4 hr as needed. Childn ≥5 yr Status epilepticus & severe recurrent convulsive seizures 1 mg slow IV every 2-5 min, may be repeated in 2-4 hr as needed. Max: 10 mg. Tetanus 5-10 mg IM/IV every 3 or 4 hr as needed.
Drug interaction
Increased CNS depressant effects w/ alcohol or other CNS depression-producing medications. Delayed absorption w/ antacids. Enhanced metabolism w/ antiepilepics eg, carbamazepine, phenobarb & phenytoin. Delayed elimination & increased plasma conc w/ cimetidine, estrogen-containing OCs, disulfiram or erythromycin. Potentiated hypotensive response w/ mecamylamine or trimethaphan. Increased plasma conc w/ INH. Prolonged t½ of ketamine. Decreased therapeutic effects of l-dopa. Prolonged elimination w/ omeprazole. Enhanced elimination w/ rifampin. Displacement from plasma-protein binding sites w/ Na valproate. Counteracted sedative & psychomotor effects w/ theophylline. Increased risk of resp depression, profound sedation, coma & death w/ opioids, clozapine, fentanyl derivatives.
