Alfentanil HCl
Indication
As anesth induction agent; as an opioid analgesic in general or adjuvant to regional anesth & for both short & long surgical procedures.
Instruction
Anesth induction agent IV bolus of ≥120 mcg/kg. Short procedures & use in outpatients IV bolus of 7-15 mcg/kg will last for <10 min procedures. Further increments of 7-15 mcg/kg should be given every 10-15 min for procedures >10 min. Procedures of medium duration 10-30 min: 20-40 mcg/kg; 30-60 min: 40-80 mcg/kg; >60 min: 80-150 mcg/kg. Maintain by either increments of 15 mcg/kg or at infusion rate of 1 mcg/kg/min until 5-10 min before completion of surgery. Long procedures Optimum analgesia & stable autonomic conditions are maintained through an individually adapted initial IV dose by adjusting the infusion rate to the severity of the surgical stimuli & reactions of the patient.
Drug interaction
Barbiturates, benzodiazepines, neuroleptics, general anesth & other nonselective CNS depressants (eg, alcohol) may potentiate resp depression of opioids. Metabolism may be inhibited by fluconazole, voriconazole, erythromycin, diltiazem & cimetidine. May increase the risk of serotonin syndrome w/ serotonergic agent eg, SSRIs, SNRIs or MAOIs. Propofol.
